Two siblings were brought to the emergency treatment unit with profuse watery diarrhoea. They were both conscious but dehydrated on admission. Acute food poisoning was diagnosed and they were fluid resuscitated. Their clinical condition improved with supportive care. However, 48 hours after admission their urine output declined and were noted to be jaundiced. They had raised creatinine and liver enzymes.

On further questioning it was revealed that they both had eaten some mushrooms picked from the jungle on the day of the admission.
